Abstract

Surunkali bel og‘rig‘i (SBO) butun dunyo bo‘ylab millionlab odamlarni ta’sir qiluvchi, sog‘liqni saqlash tizimiga katta yuk yuklaydigan va iqtisodiy zararlar keltiradigan eng dolzarb muammolardan biri hisoblanadi. Ushbu kasallik hayot sifatini sezilarli darajada pasaytiradi, ish qobiliyatini cheklaydi va ko‘pincha nogironlikka olib keladi. Ushbu maqolada restorativ neyrostimulyatsiya (RN), xususan ReActiv8 tizimi, SBO bilan og‘rigan bemorlarda statsionar (kasalxonada) va ambulatoriya (uy sharoitida) sharoitlarida qo‘llanilishi batafsil ko‘rib chiqiladi. Tadqiqot maqsadi - xalqaro klinik tajribalarni, jumladan ReActiv8-B, ReActiv8-A va ReActiv8-RESTORE sinovlarini tahlil qilish orqali RN ning samaradorligi, xavfsizligi, uzoq muddatli natijalari va bemorlarning hayot sifatini yaxshilashdagi rolini baholashdir. Adabiyotlar tahlili va klinik sinovlar ma’lumotlariga asoslanib, RN multifidus mushaklarini tiklash orqali og‘riqni 50-80% ga kamaytirishi, Oswestry Disability Index (ODI) ko‘rsatkichlarini 15-25 punktga yaxshilashi, EQ-5D-5L hayot sifati ko‘rsatkichlarini oshirishi va opioid dori vositalarining ishlatilishini 60-80% ga qisqartirishi ko‘rsatilgan. Statsionar sharoitlarda RN tez tiklanishni ta’minlaydi, chunki implantatsiya jarrohlik yo‘li bilan amalga oshiriladi va dastlabki monitoring kasalxonada o‘tkaziladi, bu esa infeksiya va asoratlar xavfini minimallashtiradi. Ambulatoriya sharoitlarda esa uzoq muddatli monitoring va o‘z-o‘zini nazorat qilish imkoniyati mavjud bo‘lib, bu bemorlarning kundalik hayotiga moslashishini osonlashtiradi. Xalqaro tajribalar, jumladan AQSh, Yevropa Ittifoqi, Avstraliya va Osiyo mamlakatlaridagi klinik sinovlar natijalari RN ning SBO davolashda yangi paradigma ekanligini tasdiqlaydi, ammo qimmatligi, implantatsiya xavfi va individual bemor xususiyatlariga bog‘liq cheklovlar mavjud. Ushbu yondashuv bemorlarning hayot sifatini oshirishda, nogironlikni kamaytirishda va sog‘liqni saqlash xarajatlarini optimallashtirishda muhim rol o‘ynashi mumkin. Maqola natijalari klinik amaliyot uchun tavsiyalar beradi va kelajakdagi tadqiqotlar uchun yo‘nalishlarni belgilaydi, masalan, RN ning boshqa neuromodulyatsiya usullari bilan taqqoslanishi va uzoq muddatli (5-10 yillik) natijalarini o‘rganish.
