Abstract

Invaziv lobulyar karsinoma (ILK) sut bezi saratonining ikkinchi eng keng tarqalgan shakli bo‘lib, sut bezlarining lobulalarida paydo bo‘lib, atrofidagi to‘qimalarga tarqaladi va gistologik darajada E-kadherin molekulasining yo‘qolishi bilan ajralib turadi. Ushbu tezis ILK ning klinik ko‘rinishlari (ko‘krak shaklining o‘zgarishi, massa shaklidagi shish yo‘qligi, teri va nipel o‘zgarishlari), morfologik xususiyatlari (yakka hujayrali infiltratsiya, stromal reaksiya, variantlar: klassik, pleomorf, alveolar, solid va tubulolobulyar), biomarkerlari (ER/PR musbatligi, HER2 ekspressiyasi, Ki-67 indeksi) va ularning prognozga (omon qolish darajasi, metastaz chastligi va joylari, relaps ehtimoli) ta’sirini batafsil ko‘rib chiqadi. Klinik sinovlar va epidemiologik tadqiqotlar shuni ko‘rsatadiki, ILK invaziv duktal karsinoma (IDK) ga nisbatan boshqa metastaz naqshlariga ega bo‘lib, ko‘proq oshqozon-ichak trakti, peritoneum, suyak va miya metastazlari bilan bog‘liq, ammo erta bosqichlarda aniqlash, maqsadli terapiya (gormonal va immunoterapiya) va jarrohlik usullari orqali prognoz sezilarli darajada yaxshilanadi. Bundan tashqari, pleomorf va solid variantlar yuqori agressivlik va yomon prognoz bilan tavsiflanadi, shuningdek, familiya tarixi va genetik mutatsiyalar (CDH1 geni) rolini o‘rganish muhim. Ushbu ish sut bezi saratonini davolash va diagnostika sohasidagi ehtiyojlarni qondirishda ILK ning yuqori dolzarbligini ta’kidlaydi, chunki bu tur kasallikning 10-15% ini tashkil etib, skrining dasturlarida qiyinchiliklar tug‘diradi va shaxsiylashtirilgan davolash strategiyalariga yo‘naltirilgan kelgusidagi tadqiqotlarga batafsil tushuncha beradi, jumladan, molekulyar profil va AI asosidagi diagnostika.
